About Misoprost®

Misoprost® is manufactured by Cipla. Misoprost® contains an active ingredient Misoprostol, which is miscellaneous GI agent – a synthetic analog of prostaglandin E1.


- NSAID-induced ulcer prophylaxis in patients receiving NSAIDs and at high risk for gastric ulceration (e.g., elderly, past history of ulcer, concomitant debilitating disease, or concomitant systemic corticosteroid use);

- kidney transplant rejection prophylaxis in patients who are currently receiving cyclosporine and prednisone;

- pregnancy termination in combination with Mifepristone, up to 49 days (7 weeks) gestation dated from the first day of the last menstrual period;

- cervical ripening induction or labor induction following third-trimester intrauterine fetal death;

- prevention and/or treatment of postpartum bleeding;

- medical management of early pregnancy failure.

MOA (mechanism of action):

Misoprostol is a potent inhibitor of gastric acid secretion in humans. It also causes the cervix to soften and the uterus to contract. Prostaglandin E1 causes muscle contraction. By interacting with prostaglandin receptors, Misoprostol causes the cervix to soften and the uterus to contract, resulting in the excreting of the uterine contents.


Recommended dose for NSAID-induced gastric and duodenal ulcers treatment and prophylaxis:

Misoprost® tablets should be taken for the duration of NSAID therapy: 200 mcg orally 4 times a day after meals and at bedtime.

Recommended dose for labor induction:

25 mcg vaginally every 4 to 6 hours.

Recommended dose for prophylaxis of postpartum bleeding:

400 to 600 mcg orally or rectally after delivery of the baby, but before delivery of the placenta.

Recommended dose for cervical ripening:

Before surgical abortion: 400 mcg vaginally, 3 to 4 hours before suction curettage.

Recommended dose for abortion taking Misoprost® only:

Four tablets are recommended and four more may be required for its completion.

It is best to use Misoprost® within 9 weeks since the last menstruation; that is, fewer than 63 days counting from the first day of the last regular period.

The earlier in the pregnancy Misoprost® is administered the better because it is safer, more effective and less painful.

Misoprost® can be used later in pregnancy but the risks of complications are higher.

Try not to use Misoprost® pills alone for second-trimester abortions, as the chances of serious complications such as uterine rupture or hemorrhage rise as pregnancy advances. Ready access to emergency care in a medical facility is essential, and women should not attempt an abortion alone. They must also be prepared for the passing of a fetus and placenta at this stage.

Women with an intrauterine contraceptive device (IUD) in place should have it removed before using Misoprost®.

Note, that Misoprost® pills are less effective alone than when combined with Mifepristone.

Recommended dose for abortion taking Misoprost® and Mifepristone:

The first trimester of pregnancy: 400 mcg orally once as a single dose 48 hours after Mifepristone administration.

Alternatively, 800 mcg vaginally 48 hours after Mifepristone administration. When used with methotrexate, 5 to 7 days later give 800 mcg vaginally (Misoprost® dose may be repeated 24 hours later if needed).

In failed pregnancy or fetal death: 800 mcg vaginally once or twice (doses given 24 hours apart).

The second trimester of pregnancy: 600 mcg vaginally, 36 to 48 hours after Mifepristone administration, followed by 400 mcg orally or vaginally every 3 hours to a maximum of 5 doses in the first 24 hours.

Do not share this medication with another person.

General Advice:

It is best to drink plenty of water throughout the whole process of taking the Misoprost®.

Eat a light food before taking pills from the Misoprost®. This helps to prevent nausea.

Plan to take Misoprost® pills at home or at any other private and safe place, where you feel comfortable.

It is best to have somebody nearby to help take care of you or make arrangements in case you need a medical assistance.

Take anesthetic when experiencing cramping pain, like Ibuprofen (200 mg x 4 times every six hours).

Seek medical emergency or contact your healthcare provider immediately if you took Misoprost® for abortion and:

- You don't bleed or have a light bleeding followed by severe pain (particularly on the right shoulder) - this may be a sign of ectopic pregnancy;

- You have an abnormal bleeding that doesn't stop after 1 or 2 hours;

- You have fever of over 38 degrees Celcius more than 24 hours after taking Misoprostol;

- You have an extreme pain that doesn't get better and is not relieved by Ibuprofen.

Missing a dose

Use Misoprost® for medical termination of pregnancy or legal abortion only when needed but not more than once in a monthly cycle. If you use Misoprost® for the NSAID therapy, take it as soon as you remember about your missing. If it is the time for the next dose you should continue your regular dosing schedule. Do not exceed the doctor's recommended dose. Do not take a double dose of Misoprost®.


If an overdose occurs and you are feeling unwell, you should seek emergency medical attention or contact your healthcare provider immediately. Misoprost® is not for use more than once in a monthly cycle (in case of medical termination of pregnancy or legal abortion).


Store at room temperature between 15 and 30 degrees C (59 and 86 degrees F) and away from excess moisture and heat. Throw away any unused medicine after the expiration date. Keep out of the reach of children.

Side effects

The most common side effects associated Misoprost® are:

  • constipation
  • flatulence
  • headache
  • indigestion
  • stomach pain
  • vomiting
  • diarrhoea

Side effects occurrence does not only depend on the medication you are taking, but also on your overall health and other factors.


Misoprost® is not for use in women under 18 years of age.

Misoprost® for abortion can be used only up to 20-th week of pregnancy.

Do not take Misoprost® for NSAID-induced gastric and duodenal ulcers treatment, if you are pregnant as it may cause birth defects, abortion (sometimes incomplete), or premature labor.

Do not take Misoprost® if you are allergic to its components or to any other prostaglandins, to the medications with the content of Misoprostol (e.g., Cytotec, Arthrotec).

Do not take Misoprost® if you plan to have a baby or breastfeeding.

Do not take Misoprost® if there is a possibility of an ectopic (or extra-uterine) pregnancy or if you had an ectopic pregnancy previously.

Do not take Misoprost® if you have a history of disease conditions: liver, heart, kidney disease, inflammatory bowel disease (IBD), irritable bowel syndrome (IBS), diabetes, anemia, problems with clotting of blood, abnormal blood pressure or cholesterol or suffer from malnutrition.

Do not take Misoprost® if you have an intrauterine contraceptive device (IUD) (e.g. the coil or the progesterone type). Get it removed before using the Misoprost®.

Do not take Misoprost® if you are dehydrated.

Do not drive or operate machinery after taking Misoprost® as it may cause dizziness.

Do not smoke while taking Misoprost®.

Do not consume alcohol while taking Misoprost®.

Misoprost® is not a method of long-term contraception.

Misoprost® doesn't protect you and your partner from sexually transmitted diseases (HIV and the others).

Keep Misoprost® away from children and don't give it to other people for using.
